Contact details (phone and email)
Emergency contact
GP details
Bank details (only if refunds are required)
I also keep brief, non-specific session notes to support the therapeutic process.
Why This Information Is Needed
Your information is used for:
Arranging or changing appointments
Sending relevant session information
Safeguarding and ensuring your safety during therapy
How Your Data Is Stored
Session notes are coded and kept separate from identifying details.
All electronic data is stored on a password- and fingerprint-protected MacBook.
I take appropriate steps to keep your information secure and confidential.
When Information May Be Shared
Your information will remain confidential unless:
There is a serious risk of harm to you or others
I am legally required to share information (e.g., court order, child protection, terrorism, fraud, money laundering, or human trafficking disclosures)
How Long Information Is Kept
Contact details: 1 year after counselling ends
Session notes: 3 years after counselling ends (insurance requirement)
After this time:
Paper records are shredded
Electronic records are securely deleted
Your Rights
You have the right to:
Request access to your data
Request corrections to inaccurate information
Request deletion of data where legally applicable
